CREDITOR MATRIX SPECIFICATIONS

  1. All creditor matrix files must be provided to the court as an ASCII DOS text file.
  2. Margins (top, bottom, left, right) should be at least one (1) inch.
  3. Each creditor must be separated by at least one blank line.
  4. Do not include page numbers, headers, footers, etc.
  5. All entries should conform to the style specifications in the Style Guide.
  6. The name and address of each creditor must be five (5) lines or less.
  7. Each line may contain no more than 40 characters including spaces.
  8. Names and addresses should be left justified (flush against the left margin, no leading spaces.)
  9. Account numbers or "attention" lines should be placed on the second line of the name / address.
  10. City, state and ZIP code must be on the last line.
  11. There must be a comma placed immediately after the city name.
  12. All states must be two-letter abbreviations.
  13. Nine-digit ZIP codes must be typed with a hyphen separating the two groups of digits.
